South Side Health Equity Collaborative provides a wide range of integrated services designed to meet the physical, mental, and social health needs of the communities we serve. Our care model is rooted in equity, innovation, and deep community partnerships.
SSHEC provides equitable, community-based care for chronic conditions like diabetes and hypertension. Through education, home monitoring, and access to specialists, we help South Side residents take control of their health.
Comprehensive, equity-focused women’s health programs with a strong emphasis on maternal health and wellness are offered by SSHEC. Our services are designed to address both the clinical and social needs of women in our community.
We provide a wide range of community-based behavioral health services, focusing on mental health, substance use disorders, and coordination of care.
We’re building career pathways in healthcare for South Side residents, starting from high school through job placement. With training, mentorship, and support, SSHEC is committed to helping our communities grow into the workforce they deserve.
We’re passionate about lifting up our community’s youth by providing safe spaces and mentorship, wellness services, and exciting programs in academics, arts, and athletics. With year-round support, we help young leaders grow, embrace healthy habits, and build brighter futures
